EPSDT Care for Kids Coordinators

An EPSDT Care Coordinator is available in every county in Iowa. Care coordinators help families of children from birth to age 21 access the health care they need. Care coordinators can also help health care providers identify local resources for developmental services for children at risk. EPSDT Care Coordination is provided by Iowa Child Health (Title V) agencies.

EPSDT Care Coordinators:

Inform families about EPSDT services and preventive care and its importance in their child’s life

Contact families when EPSDT periodic screenings are due

Help health care providers make referrals for additional developmental screening or other intervention services as needed.

Help connect families to referral sources

Link families to community resources, including translators and child care

Assist families to overcome barriers as they negotiate the health care system

Monitor each child's progress

Enable families to become more effective health care consumers

Provide TRANSPORTATION for families to medical and dental care providers who offer Medicaid services. To arrange transportation:
- In-town: Contact an EPSDT Care Coordinator in your area. Locate your coordinator by calling the Healthy Families line, 1-800-369-2229.
- Out-of-town: Contact your DHS office. Their phone number is listed in the government section of your local phone book.
