Discuss concern with weight and begin a 1-3 minute positive conversation with patient/family by assessing their readiness/ willingness to have a conversation with you to discuss ways to stay healthy.

*Materials work best when handed to patients and/or families during an office visit by provider and used to encourage them to visit Eat and Play the 5-2-1 Way. For patient education materials and tips on beginning positive conversations about weight, visit Eat and Play Health Care Resources.

 


Somewhat-very

Provider:

1. Share 5-2-1-0 and provide health contract.

2. Discuss ONE action idea from the contract that patient/family would like to try.

3. Complete and sign health contract with patient/family.

4. Share brochure(s) and encourage patient to visit Eat and Play the 5-2-1 Way.

5. Refer patient to community resources (i.e., dietitian) if desired.

6. Ask patient to schedule follow up appointment if desired.

Not ready/willing

 Provider:

1. Acknowledge and respect patient’s choice they have made today.*

2. Give patient “Nutrition and Physical Activity Tips for a Healthy Family” brochure and share Eat and Play the 5-2-1 Way, listed on the back of the brochure, as a nutrition and physical activity resource they may want to use in the future.

3. Briefly and positively review your concern but share that you can be a resource when they are ready for change.
